The Hometown Plan is designed for people who want more convenient access to Cornerstone without needing a fully all-inclusive membership. It’s a comfortable middle ground for those who need care occasionally throughout the year and appreciate being able to email their provider when questions or concerns arise.

  • Priority scheduling when an in-person appointment is needed

  • Extended visits and complete physicals for $75
